Obituary Submission Form

Obituaries will be accepted by The Gazette until 4 p.m. for publication the following day. An earlier deadline may be used for holidays. Please e-mail your obit to obits@sourcemedia.net , complete and submit the form below, or mail the corresponding information from the form to:

Pictures may be emailed to obits@sourcemedia.net Cost for each photo is $10. Please use .jpg format with a resolution of at least 200 dpi. Please name the file using the following format:

Prepayment is required before an item is printed. Please call to provide payment information upon submission. Cost is 50 cents per word and $10 per picture. Name of funeral home or crematory and respective phone number is required to verify death.
